Case Coordination Unit
PLOWS Council on Aging is a Case Coordination Unit, as such we arrange community based
services for older persons in our service area who need help. We are not only aware of
services for older persons in our communities served, but we will make arrangements to
have services provided by appropriate agencies.
As a Case Coordination Unit we will determine the older persons eligibility, inform them
if any fees are involved, see that services are provided by appropriate agencies and we
make an effort to follow up on any further needs of the client. This is done based on
guidelines required by the State of Illinois Department on Aging.
If you want to know about services for older person in the Palos Lemont, Orland or Worth
township areas call the PLOWS Council on Aging at (708) 422-6722. We can tell you about
services ranging from home delivered meals and shopping assistance to in-home care, and
adult day care. If you are not within our service area you may contact the Illinois
Department on Aging at 1-800-252-8966 and they will assist you in locating the name of
your Case Coordination Unit.
