Community Care Program

The Community Care Program is designed to assist you in remaining more independent, in your own home, and to keep you out of a nursing home, if nursing home placement is inappropriate. PLOWS will send a casemanager to your home to discuss your needs and to determine whether you qualify for the Community Care Program (CCP). The services that may be provided through the CCP program are Homemaker, Adult Day Care and Senior Companion. PLOWS will work with you to determine your needs, set up services that are appropriate for you, and keep in touch with you to make sure the services continue to meet your needs.

You are eligible for this program if you are 60 years of age or older, are determined physically in need of service, a United States Citizen or legal alien and meet the asset requirements.

If your family income is below the federal poverty level, the State of Illinois will pay the entire cost of service; you will not be charged a fee.

If your family income is above the federal poverty level, the State of Illinois may pay part of the cost of service. Your share of the cost will depend upon your level of impairment, the cost of care, and the number of people in your family who will receive the CCP services.
